Hight Health is the world leader in coordinating access to care for underserved communities. To date, Hight Health has provided over 20 free virtual and in-person, social determinants of health-related (SDOH) events. These events have spanned six cities across the Atlanta, Georgia area, covering an approximate target population of 1,010,459 Georgia residents. All the events hosted or attended by Hight Health target Health Professional Shortage Area (HPSA) designations, including low-income, Medicaid-eligible, migrant, homeless, and medically underserved Areas/Populations. SDOH data is collected from all communities served by Hight Health and used for the development of sustainable solutions.

Our team at Hight Health is proud to have hosted several successful health events, including the Inaugural City of East Point and Morehouse School of Medicine Health Equity Summit. This event centered around promoting health equity and creating a space for collaboration and innovation.

Our recent project, the Healthy Heart Ambassador Blood Pressure Self-Monitoring Program, funded by the Georgia Department of Public Health, was a huge success. We partnered with the Forest Park Ministers Association and JenCare Senior Medical Center to provide this program to the community, and we are proud of the impact it has made.

Our team recently hosted the second annual City of East Point and Morehouse School of Medicine Health Equity Summit, bringing together a diverse group of community members and professionals to discuss and address health disparities in our community.
